VNA of CORDELE

WHO WE ARE AND WHAT WE BELIEVE

It is the policy of the Visiting Nurses Association of Cordele to provide in writing to its patients and clients basic information concerning its philosophy and mission as a provider of home care services.

We are duly licensed by the state of Georgia to provide home health services including skilled nursing, physical, speech and occupational therapy, and home health aide services. We are both Medicare and Medicaid certified to provide these services and are regulated by

the Centers for Medicare and Medicaid Services (CMS) of the federal government and by the Office of Regulatory Services of the state of Georgia.

VNA of Cordele, Inc. is a non-profit corporation and is governed by a voluntary community based Board of Directors. Our main office is located in Cordele, Georgia. Branch offices are located throughout the agency’s service area in order to better serve its patients and clients. VNA is authorized to provide needed home health services in the following counties: Bibb, Crisp, Dooly, Houston, Lee, Macon, Peach, Pulaski, Schley, Sumter and Wilcox. It is our mission to promote the optimal quality of life of the individual patient through the provision of caring and professional home health care related services.

Home care is our business and our only business.

We are committed to being patient-advocates. Therefore, in addition to those rights set forth by the “Statement of Patient Rights”, we believe that our patients also have the following rights and pledge our commitment to the following:

• We will respect and support the patient’s right to pain management and will provide appropriate pain assessment/reassessment as applicable.

• We will respect and support the patient’s right to instruction and education in order to enable them to achieve optimal wellness and self-care and to know when and how to summon assistance in an emergency.

• We will respect and support the patient’s right to expect the agency to have the proper resources to render safe care of the frequency and intensity ordered by their physician. If we are unable to provide the patient with the proper care and/or services due to our own limitations, the patient will be informed before care is initiated.

• We will respect the patient’s right to refuse care and to be informed of the possible health consequences of this action and be assured that such refusal may be made without fear of reprisal or discrimination.

• We will respect the patient’s right to recommend changes in policies and services to the agency, free from restraint, interference, coercion and
discrimination or reprisal.

• The agency will respect the patient’s right to request records and bills pertaining to them and their right to challenge them, have their records corrected for accuracy, and have all such records in the case of continuing care with another facility.

• The agency will respect and support the patient’s right to coordination and continuity of care and the right to supervision of the home health aide by a Registered Nurse or appropriate professional and to supervision of a therapy assistant by a qualified therapist.

• We believe that the patient has the right to participate in investigational protocols, research or clinical trials and since as an agency, we have determined that we will not participate in these, we will assist the patient and the attending physician in transferring the patient to another entity for the provision of care making the transition as smoothly and timely as possible.

We believe that we exist for only one reason, to meet the health care needs of those we serve, our patients. We believe that we can do no less than our very best to provide the very highest quality of home care services in accordance with the plan of care established by the physician.

VNA of Cordele, Inc. is accredited by the Joint Commission.  Any individual with concerns about patient care and safety provided by the organization, that the organization has not addressed, is encouraged to contact the agency’s Director of Clinical Services at 800-334-8268.  If these concerns cannot be resolved through the organization’s management, the individual is encouraged to contact the Joint Commission.  The public may contact the Joint Commission’s Office of Quality Monitoring to report any concerns or register complains about a Joint Commission-accredited health care organization by either calling 1-800-994-6616 or emailing complaint@jcaho.org
